In order to implement QI initiatives it is necessary to look at tools which will aid with this process. This includes the assessment of data which is needed in order for each improvement to take place as well as the identification of the tools which may be used to assess and measure changed and the way that they measure those changes (Pyzdek, 2001). There is different data required for measurement of these two variables, with the comparison is required in order to determine whether or not there is any improvement taking place. The data required not only reflects the need to measurement, the data also will be required in order to help explain the measurements that are taking place, and provide information on the underlying factors that are impacting on those measurements. For example, the basic measurement that is required in order to assess whether or not there is a reduction in number forms will be the number of falls per patient, or the frequency level of falls in terms of overall patient numbers. For quality improvement to take place it is necessary not only to measure the number of falls, but also gather data which may indicate why the falls are taking place, in order to determine the measures which may be needed in order to improve performance in this area. A similar approach is needed in terms of the data collected when assessing hospitalisations in chronically ill patients.
