The term "error" might be defined as an action that leads to and unintended and unanticipated outcome. In the healthcare arena errors can be life threatening. At the very least they can be counterproductive to the healing process. The reasons behind errors, whether minor errors or potentially catastrophic ones, are essentially the same in regard to human performance. There is, of course, a distinction between true errors and violations where an individual intentionally disobeys established protocol or procedure or unintentionally does so resulting in an error. The outcome, however, can be just as catastrophic in both cases. Because of the potentially serious retributions of errors in health care a number of protocols have been put in place to eliminate, or at least to minimize, them. Despite the protocols we have in place, errors are something that continue to occur on a much too frequent basis. A nurse might make an error in the amount or type of medicine she administers, for example. A physical therapist might unintentionally require a patient to perform a movement that could be disastrous in terms of their recuperation. Doctors themselves, on the other hand, are certainly not immune from making errors. Everyone has heard horror stories of doctors amputating the wrong limb or even removing an organ from the wrong patient! These are the types of errors that make national news. Most errors in the medical environment, however, are less phenomenal in their presentation.
