Psychologically, it has been found that individuals more actively involved with their own health care often fare much better than those dependent on and at the mercy of the medical community. Practically, self-care is crucial in the management of diabetes, which now is at "epidemic" levels in the United States. The purpose here is to observe a nurses instruction on checking blood glucose before taking insulin, and then assessing the instructional effort. The Nurses Instruction The nurse instructed a diabetes patient in how to take his blood glucose before taking insulin. The patient has been striving to control his diabetes with diet and exercise for some time, but the condition has progressed to the point that he now needs to add insulin injections to his daily routine. The nurse stressed the importance of taking blood glucose before taking any insulin at all, explaining that in the early stages of adding insulin, it may be necessary to adjust dosage and that it is critical that the patient check his blood glucose regularly and record the values obtained. The nurse instructed the patient to take insulin only when his blood glucose level was above the value established by his physician. The nurse laid out all of the supplies and equipment that the patient would need to complete the task, placing them in a line in order of use and explaining in turn why each is needed.
