etc., offend the therapist personally. This paper summarizes material about a case study of just such a relationship, and an honest response to it. Discussion In the case study, the client, Betty S., is overweight, has had three illegitimate children by three different men, remains unmarried, and lives with an unsympathetic aunt who has relegated her to a room in the garage where she spends most of her time sleeping. Her "apartment" is filthy, and when the social worker first sees her, she is still in bed, lying on sheets that are grey with dirt. There is so much clutter in the place that the social worker has to move things in order to find a place to sit down. The immediate reaction of a reader to this description is likely to be one of distaste, dislike and perhaps even outright disgust. Its all too easy to imagine the stench of the womans unwashed body, the grimy sheets, the dirty dishes and spilled food, and the infestation of bugs and rodents that is likely to follow. The social worker would be hard pressed not to take an immediate dislike to the woman, followed by an almost uncontrollable impulse to dunk her into a tub of bleach and take a scrub brush to her, then hose down the apartment. People with poor personal hygiene, not to put to fine a point on it, stink; human body odor is extremely offensive and can literally be sickening. The first task for the social worker then is to overcome revulsion for the womans physical situation and that is going to be difficult.
