order to assess the current perspectives on the usefulness of proning in patients with ARDS, it is useful to first consider briefly the ways in which ARDS itself is manifested, since it is a condition which can be associated with a number of different diseases. Basically, Acute Respiratory Distress Syndrome, or ARDS, is a severe acute injury which has occurred to both lungs. ARDS patients suffer from severe shortness of breath, usually to the extent that they require mechanical ventilation to maintain respiration. ARDS can occur in association with various conditions, such as pneumonia, shock, infection, and trauma; it is also sometimes confused with congestive heart failure, which has a similar impact on the respiratory system. ARDS is a progressive and potentially fatal condition, as the damaged lungs fill with inflammatory cells and formation of scar tissue takes place: this fibrosis further hinders gaseous exchange, which has already been limited by damage to the alveoli in the early stages of the condition. The causes of ARDS are not fully understood, but there are two main types of injury which can cause it. In the first, there is a direct injury to the lungs, caused for example by smoke inhalation, inhalation of vomit, or a direct blow to the chest. In the second, which as Hansen-Flaschen (2004) points out, is more common but not as well understood, there is an indirect injury to the lungs.
